 With your application you must provide proof of identity, proof of residency and proof eligibility. A minimum of two separate documents are required as proof i.e. you cannot use a driving licence as proof of all of the above. Please note:- we can accept photocopies – please do not send originals.
For Proof of identity we need to see:
Birth certificate, passport, NHS medical card, driving licence or statement of entitlement to a pension.
For Proof of residency we need to see:
Bank statement, benefit book, council tax bill, driving licence, doctors letter, letter or statement from HMRC, medical or optical prescription, NHS medical card, pension book or utilities bill (not credit card or mobile phone).
Proof of Eligibility
| People aged 60 or over | Birth certificate, driving license, passport, senior railcard, winter fuel payment letter, pension credit or statement of entitlement (from DWP Pension Service) |
| 1 People who are registered blind/partially sighted | Evidence of registration with Social Services |
| 2 People who are severely or profoundly deaf | Evidence of registration with Social Services, or a report from an aural Specialist |
| 3 People who cannot speak | Medical evidence/Doctor’s letter |
| 4 People who have a disability or have suffered an injury which has a serious long-term effect on their ability to walk | Proof that you are receiving the higher-rate mobility component of DLA, or medical evidence |
| 5 People who are without the use of both arms | Medical evidence/Doctor’s letter |
| 6 People who have a learning Disability | Evidence of registration with Social Services, or medical evidence |
| 7 People who are prohibited from driving a motor vehicle on medical grounds | Medical evidence/Doctor’s letter |
