How do I surrender a Personal Licence?

If you would like to surrender your Personal Licence you are required to complete the  Surrender of a Personal Licence Notice form (PDF) [160KB] (opens new window) .

You should also return your Personal Licence, if you are unable to do that you must provide the reason why.
