Notes for businesses:

As the food business operator of the establishment, you have a right to request a revisit for the purposes of re-rating if you have taken action to rectify the non compliance identified at the time of the inspection.
A food business may apply for a re-visit at any time and as often as they choose.
A fee of £210 is charged for each request which must be paid before the revisit takes place.
You must provide details of the improvements made with your request, including supporting evidence where appropriate.
The revisit will take place within three months of the request and receipt of payment. The local authority officer will give you a 'new' food hygiene rating based on the level of compliance that is found at the time of the revisit - you should be aware that your rating could go up, down or remain the same.

Address for sending cheque is: Food and Health & Safety Team, Breckland Council, Elizabeth House, Walpole Loke, Dereham, NR19 1EE. You will need to provide details with the cheque, including the name of the business and that you are applying for a revisit for Food Hygiene Rating.
